Required Daily Student Health Screening

We're excited to welcome your child back to the classroom.  To help us all stay safe and healthy we have implemented a new daily health and safety screening process.

Parents and guardians MUST complete one for each of your children before they arrive at school every day.

Nos complace darle la bienvenida a su hijo al salón de clases.  Para ayudarnos a todos a mantenernos seguros 7 saludables, hemos implementado un nuevo proceso diario de detección de salud y seguridad.

Los padres y tutores DEBEN completar una evaluación de salud para sus estudiantes antes de que lleguen a la escuela todos los días.

STEP 1.  Complete the online health screener.  The primary guardian in each household will be emailed a link (date specific)  to screen their student each day before they come to school.  A guardian can also request a text message be sent to a different number (for instance the other parent or guardian who would need to screen the student prior to getting on bus or coming to school).  The priimary guardian will still get the daily email as will the second parent/guardian via text. OR you can; 

STEP 2.  Complete the home health screen form on paper(ENG/SPAN).  Your building office can get you the form. 

STEP 3.  The parent would show the attestation screen on their phone to bus driver or the school.  Or if you do the paper form each day - give that to your student to show bus driver or at school every day.
