Medications at School
All medications given at school/sports/activities/field trips, etc. must be accompanied by a Medication Form signed by a physician/licensed practitioner as well as the parent.
These forms are available at all school offices. This procedure applies to all medications, including over-the-counter as well as prescription, whether oral medication, inhalers, injectable Epipens and insulin.
All medications must be in the original/labeled container with student name, date, and dosage instructions specific for school only. Medications must be brought to school by a parent/guardian and will be kept locked in the office.
These forms are available at all school offices and must renewed each fall. If you have any questions please contact the school nurse, Kesha Smith or Shelly Zehm, through your school office.
