All medications given at school/sports/activities/field trips, etc. must be accompanied by a Medication Form signed by a physician/licensed practitioner as well as the parent.

These forms are available at all school offices. This procedure applies to all medications, including over-the-counter as well as prescription, whether oral medication, inhalers, injectable Epipens and insulin. 

All medications must be in the original/labeled container with student name, date, and dosage instructions specific for school only. Medications must be brought to school by a parent/guardian and will be kept locked in the office.
